WHO IS THE PERSONAL CARE ADVOCATE
NURSE? Every
House Call Doctor has a nurse to assist him or her. Your Personal Care Advocate Nurse works behind the scenes, calling you
regularly to see how you are doing and to make sure you are getting the care you need. If you need a house call visit, the
nurse will set it up. And when your House Call Doctor prescribes a treatment such as in-home IV medicines or oxygen, he or
she will contact the nurse to make all the arrangements. The
nurse also: • Keeps your Office Doctor updated on your home
treatment • Helps you with services such as Meals on Wheels,
Companion and Respite Care Your Personal House Call Care Team Includes: • Board-Certified Doctor • Personal Care Advocate Nurse • Nurses and Doctor’s Assistants • Administrative Staff
